Custom Music CD's

Order custom music CD's to ship to family and friends. Simply create a playlist of nine songs and their artists. We'll create the CD and label for the jewel case that features your playlist, along with the album title you choose.
Use this order form to order a CD.

Instructions: on the order form, list each song and artist, along with two or three alternatives in case we can't locate all of your choices. You may enclose a brief, personal note (up to 25 words) to include with the CD.

Each CD costs $25.00, which includes shipping and handling. Payment may be made by facility check, money order, or 80 USPS 1st Class (or Forever) stamps.

The CD will ship to the name and address provide on the Order Form. Print out and complete a copy, send it in and we'll get started on your Custom CD!

Important note: CD's are non-refundable. If you want the CD shipped to a facility, check in advance to ensure that the inmate may receive CD's (and in what quantity).
